For parent/teacher conferences, I created this "Something to Glow on, Something to Grow on" reflection for each student (saw this idea on Pinterest last year). It helps me to organize my own thoughts about each student and it gives the parents something to take home! For individual students, I provide copies of work samples, math tests, and possible interventions to try at home!
